Hazard again hits a shot goalwards after cutting inside from the left, but his fellow countryman Courtois holds on well.
90' +4
GOOD SAVE! Hazard finally comes to life as he twists his way through the Atletico defence, but Courtois reads the shot well to save with his feet before Terry curls the rebound over. Celebrations from the goalkeeper with the defenders in front of him despite the game being safe.
90' +3
Still the Chelsea shots come in on goal but the efforts of Ba and Eto'o are closed down by the Atletico defenders. It just hasn't been the home side's evening in front of goal.
90' +1
Into five minutes of added time now, but not even that will be long enough for Chelsea to get the three goals they need to turn this around and reach the final.
89'
Simeone is still barking out instructions on the touchline but many would forgive him now for relaxing given the way his side have performed. This is yet another feather in the Argentinean's cap.
87'
Schurrle tries to create something on the edge of the box but the visitors can again neutralise the danger. The defensive quintet of Atletico are going to extend their run of not being beaten this season when all five have started together.
85'
Chelsea are still looking to push men forward but it is all for nothing. They weren't solid enough at the back this evening and errors in defence have cost them a shot at the trophy in Lisbon.
83'
C. Rodríguez enters the game and replaces A. Turan.
Finally Atletico make their change, with Turan coming off for Rodriguez. Not really sure why that didn't happen in the first place, though Filipe Luis was limping slightly just beforehand.
82'
Some confusion here as Turan is turned away as he makes his way across to the touchline, so Rodriguez will have to wait for his moment to come on.
81'
Simeone makes his final change and Turan becomes the third Atletico goalscorer to be replaced as Rodriguez is given the final ten minutes to impress.

Description
79'
Atletico are almost treating this as a training game now as they keep possession in midfield before trying to cut open the Chelsea defence. The hosts look perplexed as to what has happened in this second half.
77'
A. Schürrle enters the game and replaces Willian.
Filipe Luis almost adds a fourth as a turn of pace gets him in behind the Chelsea defence, but Schwarzer is out quickly to turn his chipped effort around the post.
76'
J. Sosa enters the game and replaces Diego Costa.
Costa eventually trudges from the field as Sosa replaces him while Schurrle is on in Willian's place for the hosts. The German inspired them to a quarter-final comeback, but to do it again tonight would be something else.
75'
Costa is limping and has eventually gone down as he looks to receive treatment. Terry and Azpilicueta help him up though as they look to keep the game moving and chase the goals they need.
74'
The Chelsea fans have been stunned into silence and the fans from Spain are having a party down in their own little corner. Since falling behind this has been an exceptional showing from Simeone's side.
72'
A. Turan has scored a goal for Atletico Madrid!
GOAL!!! That may well be that and Turan is the man to find the net! Yet more balls into the box cause the Chelsea defence problems as Hazard allows Juanfran to ghost in a the back post. Turan's first header is turned onto the bar by Schwarzer, but the Turk is able to tap home the rebound and send Simeone dancing down the touchline. Chelsea require three in less than 20 minutes.
69'
Willian's balls into the box have proved difficult to defend, and he has another chance as Garcia trips Hazard. Courtois, however, rises to collect this one and is fouled by Ba in the eyes of the referee. Atletico holding firm at present as a Madrid derby in Lisbon moves ever closer.
67'
D. Ba enters the game and replaces Fernando Torres.
And now Ba is also coming on, with Torres the man replaced. The striker is applauded from the pitch by both sets of supporters.
66'
Raúl García enters the game and replaces Adrián López.
First, though, Atletico make a change as Adrian is replaced by Raul Garcia.
65'
Adrián López gets yellow.
From the resulting corner the ball falls for Ramires but his powerful drive from distance is too close to Courtois. At the other end Adrian is booked for a late challenge on Hazard. Ba is being readied on the Chelsea bench.
64'
OFF THE POST! So close to an equaliser on the night for Chelsea as Willian's ball into the box is headed onto the post by Luiz, and with Ivanovic waiting to tap in the rebound, Courtois gets a vital touch to turn it over.
62'
Chelsea are leaving themselves rather open now, and Costa is able to break forward and win a corner after his shot is deflected wide by Cahill.
60'
Diego Costa gets yellow.
Penalty goal scored by Diego Costa for Atletico Madrid!
Some tension here as Costa is booked for trying to place the ball in a divot on the penalty spot. Here we go...
GOAL!!! Schwarzer is sent the wrong way and Costa, a man who is rumoured to be heading to Stamford Bridge in the summer, may well have scored the goal to knock Chelsea out of this season's Champions League. The Blues require two goals to win.
58'
PENALTY!!! Is this the moment for Atletico? A ball is lobbed into the box that Costa wins ahead of Eto'o, and the Cameroon man trips his opposite number. Costa himself is going to step up...
56'
Eto'o has joined Torres up front, with Azpilicueta reverting to left-back and Willian wide on the right of midfield. Little to show from it as yet but Mourinho is full of encouragement.
54'
S. Eto'o enters the game and replaces A. Cole.
Mourinho has now made that change, and with Cole making way for Eto'o, it will mean a change of system for the hosts too.
53'
GREAT SAVE! Chelsea finally call Courtois into some action as Willian whips in a dangerous free-kick from the right that Terry meets powerfully. His header is a good one but the goalkeeper gets down well to push it away.
52'
Schwarzer is again called into action as Tiago hits a low effort from the edge of the area, but the goalkeeper can get down to his left to hold the shot.
51'
Cole looks to hit the ball in Torres' direction but Courtois comes to the edge of his area to collect. Not happened since the break for the hosts and Eto'o is being readied down on the bench.
49'
Atletico are having all the play since the break, and Suarez sends another effort goalwards from 25 yards, but his shot is well off-target. Chelsea cannot afford to concede again.
47'
Early appeals for a penalty from Miranda as he goes down under pressure from Willian, but the referee is having none of it and waves the protests away.
GOOD SAVE! Chelsea's defence has been poor tonight, and they have Schwarzer to thank for not falling behind. A low cross from the right evades everyone despite Terry's attempted clearance. Turan waits at the back post before hitting a shot goalwards, but the goalkeeper stands up tall to tip over the bar.
46'
Atletico kick-off and we are back underway...
44'
Adrián López has scored a goal for Atletico Madrid! Assist by Juanfran.
GOAL!!! Atletico have responded, and now they lead the tie through Adrian! Tiago crosses from the left-hand side, and after Hazard allows Juanfran to keep the ball alive at the back post, Adrian has the easiest of tasks to tap home. The pocket of visiting fans go wild and why wouldn't they?!
42'
It's yet another excellent ball into the box which Adrian gets a flick onto, but Schwarzer watches it closely and in the end collects with ease.
41'
Chance here for Atletico as Ramires trips Turan, who has moved over to the left, and Koke can deliver this free-kick from the left-hand side...
39'
Cahill yet again gets himself in trouble as Filipe Luis crosses from the left and the centre-back totally misses his headed clearance. Costa isn't prepared for it though and Hazard wins a free-kick from the striker.
38'
The stadium is in full voice now, and Atletico need to respond. Remember, an away goal would put Simeone's side back ahead in the tie after the goalless first leg.
36'
Fernando Torres has scored a goal for Chelsea! Assist by César Azpilicueta.
GOAL!!! It just had to be that man Torres who finds the net against his former club and boyhood heroes! Willian is allowed to wriggle free of Filipe Luis and Godin, and after Azpilicueta pulls the ball back from the right, Torres slides the ball past Courtois via a Miranda deflection. No celebration from the goalscorer but what a huge moment in this tie.
34'
Atletico do seem to have taken a bit more control of this one after a period of Chelsea pressure in the opening stages. Little for Torres or Costa to feed on as yet though.
32'
Koke whips it into a dangerous area, but Terry reads it brilliantly to head away and put the hosts back in possession of the ball.
31'
G. Cahill gets yellow.
Cahill now picks up the first booking of the evening as he is beaten for pace by Costa and the England defender can have no complaints. Atletico have a free-kick 25 yards from goal to the left-hand side.
30'
Cahill has certainly been put under a bit of pressure in this first half, and twice he loses the ball allowing Filipe Luis to try and cross from the left, but Ramires is back to win a goal kick. It remains goalless in the tie.
28'
Ivanovic, despite seemingly playing in a central defensive role, has got forward down the right-hand side a few times already, and he almost creates an opportunity for Ramires, but Godin gets across well to clear.
26'
Hazard this time goes for the spectacular as a corner is cleared to him on the edge of the area, but his attempted volley lands high in the stands behind Courtois' goal.
24'
Hazard certainly looks fit as he combines with Cole on the left-hand side to win a corner. Juanfran and Turan need to get tighter to keep the PFA Young Player of the Year quiet.
23'
JUST WIDE! Almost a spectacular opener as Ivanovic's long throw from the right is controlled by Luiz, and after taking another touch he looks to hit an overhead kick that falls just wide of the post.
21'
Atletico bring the ball forward but after Koke is crowded out by Willian, Turan again concedes a free-kick. The Turkey international has struggled to stamp his authority on this one so far.
19'
Cahill allows Atletico to attack as the defender gives the ball straight to the opposition. The ball is worked to Costa but his effort from a tight angle is well blocked by the retreating Cahill.
17'
Atletico seeing precious little of the ball at the moment with the hosts playing mostly in the opposition's half. Courtois is called into action for the first time as Hazard sends in a low cross, but the Chelsea loanee can dive on the ball under little pressure.
15'
Willian is again the man to step up, but his effort is always too high and Courtois can watch it go harmlessly over the bar.
14'
Opportunity for Chelsea as poor control from Juanfran leads to Turan tripping Hazard on the left-hand side of the box. Willian plays it short to Ramires, and the Brazilian too goes down and now the hosts have a free-kick in a central area 20 yards from goal.
12'
There is certainly a high tempo to this match, but as with last week's encounter defences are very much on top. A personal mistake may be the only way either of these sides concede this evening.
10'
Almost a mix-up in the Chelsea defence as a long ball over the top looks for Costa, and with Terry and Schwarzer looking at each other, the goalkeeper just beats the striker to the ball on the edge of his box.
8'
The home fans lift the volume up a notch for the first time as Mourinho's men enjoy a period of possession in midfield. Little in the way of clear chances as yet, with Koke's effort the only shot on goal.
6'
Luiz looks to find Torres on the edge of the Atletico box but Miranda is able to use his strength to manoeuvre the Spain man away from the ball and the pass floats out for a goal kick.
4'
OFF THE BAR! Almost a disastrous start for Schwarzer as a corner is cleared back to Koke on the left, and the midfielder hits it goalward and over the Australian goalkeeper's head. Fortunately for the home side it came back off the crossbar and they can breathe again.
3'
An even start to proceedings with Chelsea seemingly having lined-up with a 5-4-1 formation. Azpilicueta and Cole will look to push on as wing-backs when the home side attack with Torres up front on his own.
1'
Chelsea kick-off and we are underway at Stamford Bridge...
Hello and welcome to live coverage of the Champions League semi-final second leg between Chelsea and Atletico Madrid. After a goalless first leg, both sides know any kind of victory tonight will see them through to the final to take on Real Madrid in Lisbon. I'm Tom Maston and I'll be guiding you through all the action from Stamford Bridge.
Chelsea boss Jose Mourinho was criticised for his defensive style of play in Madrid last week, but coming away from the Spanish capital is no mean feat. Similar tactics again paid off as they beat Liverpool at Anfield on Sunday, and though it may not be the best football to watch, he certainly knows how to win. With the carrot of taking on his former side Real in the final, he will most definitely be doing all he can to go through by any means possible this evening.
Atletico, meanwhile, have been praised for the way in which they have revitalised La Liga this campaign, with Diego Simeone's team now on the cusp of securing the Spanish league title ahead of both Real Madrid and Barcelona. Few expected them to reach this stage of the Champions League when the original group draw was made, but they now stand on the verge of a local derby in arguably the biggest game of club football in the world.
Before we go any further with the build-up, the two team line-ups have been revealed...
Mourinho has made it clear that this is going to be another defensive display, with defenders David Luiz and Cesar Azpilicueta set to play in midfield. John Terry, who seemed to be ruled out for the season after going off injured in last week's first leg, has made a miraculous recovery to captain the side, but goalkeeper Petr Cech hasn't recovered from his disclocated shoulder. Eden Hazard starts for the first time since the 2-0 win over Paris Saint-Germain in the quarter-final second leg.
Simeone has to make do without suspended skipper Gabi, so former Chelsea midfielder Tiago gets an opportunity from the start. Arda Turan also comes in while Adrian is surprisingly preferred to Raul Garcia and David Villa as a support for Diego Costa.
Atletico are the only unbeaten side in this season's Champions League, and avoiding defeat tonight would see them take the tie to at the very least extra-time. Chelsea, meanwhile, have never lost a European semi-final at Stamford Bridge, though they have been knocked out by away goals such as Andre Iniesta's last-gasp goal for Barcelona in 2009.
There are just under ten minutes until kick-off at Stamford Bridge, and though there is currently a tube strike in London, the stadium is certainly starting to fill up. The match is officially a sell-out, so expect a great atmosphere when the teams make their way out onto the pitch and that famous anthem plays.
The blue and white flags are out in full force as the two line-ups make their way out onto the field. Carlo Ancelotti and his Real Madrid players will surely be watching on intently as the two managers in West London warmly shake hands on the touchline.
Mourinho has spoken to Sky Sports ahead of kick-off, and has explained his decision to select six defenders, saying: "I cannot have a team with more than one player not in top condition. I decided on Eden [Hazard], and because we lost [Frank] Lampard and [John-Obi] Mikel, we don't have players that give us stability [in midfield]. I need to make a compact team."
The two teams are almost ready to make their entrance and the home fans are in great voice. Mourinho has made his way to the end of the tunnel as he waits for the two sides to make their arrival into the arena.
As the two teams go through the pre-match niceties, a reminder that bookings for Chelsea's Willian and Luiz or Atletico's Juanfran and Koke would rule them out of a final apperance through suspension.
There will be a minute's applause ahead of kick-off as a mark of respect for former Barcelona coach Tito Vilanova and ex-Sampdoria manager Vujadin Boskov, who both passed away recently.
If Chelsea do require a late goal, then Demba Ba is likely to be key from the bench. The striker has scored in the Blues' last three Champions League home matches, including the late goal against Paris Saint-Germain in the last round, as well as against Liverpool at the weekend.
And with that the half-time whistle blows with the tie fascinatingly poised. Torres' goal looked to have put Chelsea in control, but after Adrian's equaliser, Mourinho may be forced into attacking in the second period.
Those two goals of course mean there will be no extra-time tonight, and the tie will be decided within 90 minutes. But who will be heading through? I'll be back in ten minutes to find out.
The players are making their way out onto the field as Mourinho looks for the Italian referee to explain some of his decisions. Whether it is just mind games from the Chelsea boss I'm not sure but we are ready to get going again.
The full-time whistle blows, and though the Chelsea fans strike up a final rendition of the club song, this is Atletico's night. Hugs all round from the visiting players and they will face city rivals Real Madrid in Lisbon on May 24.
Terry is visibly upset, with former team-mate and opposing captain Tiago is the first to go console him. The former England skipper's Champions League heartache goes on.
This defeat means that Mourinho is now highly likely to go two seasons without winning a trophy, though he will certainly be given time by Roman Abramovich to go one step further next season. For once, he was out thought by his opposing manager on the big occasion.
Torres' opener against his former side seems so long ago now, with strikes from Adrian, Costa and Turan turning the tie on its head and seeing the visitors through. They thoroughly deserved it, and with La Liga title in their hands, the Atletico fans are in for quite an end to the campaign. For now, though, I've been Tom Maston, and thanks for joining me.
