Tammy Abraham hasn't scored for a while and he almost has a late chance here following a long ball forward but the young striker can't get the ball under control, which is likely to be the last action of the game.
90' +1
THREE minutes added on at the Liberty Stadium.
90'
T. Abraham enters the game and replaces S. Clucas.
Now Swansea make the change, with Sam Clucas replaced by Tammy Abraham.
89'
Swansea are set to make their final change of the match but they've been knocking the ball around well during the past few minutes.
87'
It's going to be a long journey back to east London for West Ham, who have plenty to think about. A win today would have put them within touching distance of safety but this defeat will suck them right into the thick of things at the bottom of the Premier League.
85'
Into the final five minutes, plus additional time, and the game has settled down. The writing has been on the wall for a while and Swansea have taken their foot off the gas, whilst West Ham knock the ball around without creating too much.
83'
Arnautovic flagged offside and it's not for the first time this afternoon. The Austrian has been one of West Ham's shining lights since David Moyes took charge but it hasn't been his day this afternoon.
81'
T. Carroll enters the game and replaces A. King.
Second change in quick succession for Swansea, as Andy King is replaced by Tom Carroll. King, like Ayew, has excelled on his first start for the club since his January move and his goal shortly after the restart was crucial to getting Swansea on their way.
78'
N. Dyer enters the game and replaces A. Ayew.
M. Antonio has scored a goal for West Ham United! Assist by A. Cresswell.
First Swansea change of the match, as Andre Ayew makes way for Nathan Dyer. Ayew's first start since returning to South Wales has seen him perform very well.
GOAL! Michail Antonio drives a low finish past Fabianski after he takes Cresswell's free-kick under control, swiftly turning to smash home. He's scored off the bench for the second game in a row but, like at Anfield last week, it's only set to be a consolation.
A fairly direct route for the goal, as Cresswell fired a free-kick forward from the half-way line that Antonio did superbly with.
76'
Another break in play as Antonio and Mawson clash heads inside the area, which Atkinson spots, and both players receive treatment. While the rest of the Premier League games edge towards their conclusion, we've still got a fair chunk to play due to the delay in the first half.
Minute
Description
74'
Kouyate is caught by Jordan Ayew but play continues until Swansea put the ball out of play. The West Ham man is soon back on his feet though and the hosts get the ball back, though they'll have to work their way forward again.
72'
Swansea's revival so far has been impressive and there's still a fair amount of work to be done. This result will really issue a statement of intent to the bottom of the Premier League, many of whom are struggling in front of goal. Swansea don't seem to have that problem!
70'
Arnautovic, Hernandez and Lanzini continue to struggle to get opportunities on the ball, as has substitute Michail Antonio. A leaky defence and an attack that can't get on the ball adds up to a lot of problems for the Hammers.
68'
West Ham top the table for yellow cards this season and that's another aspect of the game that Moyes needs to cut out heading into the latter stages of the season, as suspensions could really haunt his struggling side.
66'
M. Arnautović gets yellow.
C. Kouyaté gets yellow.
Frustration certainly boiling over for the Hammers, as Arnautovic comes in late on Naughton and gets his name taken.
Kouyate's bad few minutes continue as he enters the book for his latest foul.
65'
Swansea on course for their biggest Premier League win of the season and there's still time to get even more. As I said earlier, goal difference could be important come the end of the season with how tight things are at the bottom and Swansea have no desire to take their foot off the gas.
63'
Penalty goal scored by J. Ayew for Swansea City!
GOAL! Jordan Ayew converts from the spot to make it 4-0 and Swansea are cruising. He steps-up, sends Adrian the wrong way and tucks the ball into the left-hand corner of the net.
PENALTY! It goes from bad to worse for West Ham! Kouyate clips Andre Ayew inside the box, with the Hammers midfielder not aware of the presence of his former team-mate, and it's an easy decision for Martin Atkinson.
61'
West Ham are still showing some fight in attack and are putting Swansea under pressure when the Welsh side have the ball but it's still all far too easy for the hosts. The home supporters are understandably full of voice and there's every reason to be optimistic for the remainder of the season.
59'
Swansea have been in control throughout and Fabianski, when he has been called upon, has found things fairly easy.
57'
CHANCE! That just about sums up West Ham's afternoon. Arnautovic breaks through the defence and has an opportunity to shoot with his left foot but instead he switches play back onto his right, turning his marker, but his shot is easily clutched onto by Fabianski.
56'
The Hammers don't really seem to have any sort of plan and they're coming up against a very organised Swansea side. Moyes has already made two of his three changes, the first of which was forced, so it may turn into damage limitation fairly soon.
54'
West Ham are still doing better in the final third but they're yet to create anything of note yet. The latest corner from Lanzini was punched away easily by Fabianski.
52'
STAT: Bleak for West Ham, as this is now the tenth time in the Premier League this season that they've conceded three or more goals. As it stands, Swansea leapfrog them in the table on goal difference.
50'
STAT: That's now five goals in seven games for Andy King against West Ham. It certainly seems like they're his favourite opposition to face!
48'
A. King has scored a goal for Swansea City!
GOAL! Swansea make it three! The hosts win a corner as Cresswell slices the ball behind and the set-piece provides the breakthrough again. Ayew wins the initial header, which is palmed off the line by Adrian but it bounces off Hernandez and back into the middle where Andy King pokes home.
47'
A lively start from West Ham, who see Arnautovic flagged offside, but they've already shown more attacking intent in the opening few minutes of this half than they did in the entirety of the first period.
46'
M. Antonio enters the game and replaces P. Evra.
Change at the break for West Ham, their second of the game, as Evra is replaced by Antonio.
45' +10
Last chance for Swansea as they earn a free-kick on the far side but Clucas' delivery is headed away. That should be it for the half.
45' +8
Corner for West Ham, with Swansea's defensive line standing firm, and it's a rare touch of the ball for Lanzini. His delivery is poor though as it's easily dealt with by Van der Hoorn at the near post.
45' +6
Swansea still look to be the more likely of the two sides to score the next goal and, with how tight things are at the bottom, goal difference could come into effect. West Ham are fortunate to only be two behind.
45' +4
West Ham really need to buck their ideas up at the break. They've struggled to get the ball out of their defence, instead being restricted to long balls, and the attacking members of their side have simply been passengers. Arnautovic, Hernandez and Lanzini have barely had a sniff.
45' +2
CHANCE! Ki tries his luck from distance yet again, as he has plenty of room to work with, but he blasts an effort well over the top of Adrian's goal.
45' +1
TEN minutes added on at the end of the first half, following that lengthy break due to the treatment for Winston Reid.
45'
An update on Winston Reid following his early substition, as it's stated he's gone to hospital for a precautionary scan. As well as the awkward twist on his knee, he also took a big blow to the head as he hit the ground. Hopefully it's not as bad as it looked.
KICK OFF! Back underway for the second half, where Swansea get us started and now attack to the right.
43'
Swansea need one more goal to leapfrog West Ham in the table on goal difference and it shows how fantastically Carvalhal has done in south Wales. The Swans looked doomed at one point, five points adrift, but the great escape is well and truly on now.
41'
M. Noble gets yellow.
First booking of the game is issued to Mark Noble and he's unfortunate as he didn't make contact with King, although he has been combative in the middle of the field and had already attracted Martin Atkinson's attention.
40'
A rare opportunity for Arnautovic in the final third but he's lacking the support, as his cross into the middle is easily blocked and Fabianski rushes off his line to prevent the ball from going behind for a corner.
38'
Swansea remain on the front foot, although West Ham have had 60% of possession since kick-off. The majority of that has been defensively though, with Hernandez and Arnautovic at the other end struggling to get on the ball.
36'
West Ham have only won two games on the road this season and a huge turnaround is required if they're to salvage something from this game. As it stands, they're being sucked right back into the relegation tussle.
34'
West Ham have a mountain to climb now and Swansea certainly seem to be firing on all cylinders. Carvalhal's side have won six games in a row at the Liberty Stadium across all competitions and are on-course for a seventh, which would be a club-record since their move to the stadium 13 years ago.
32'
M. van der Hoorn has scored a goal for Swansea City! Assist by Ki Sung-Yueng.
GOAL! Swansea double their advantage from the resulting corner! Van der Hoorn loses Declan Rice at the far post and heads home with ease. It's only his second goal in English football but it could be a massive one for the Swans!
Ki Sung-yueng is at it again, his corner delivery is spot on and Van der Hoorn doesn't have too much to do apart from direct it at goal.
31'
CHANCE! The other Ayew in the action again, as Andre cuts inside and fires a shot from distance that Adrian needs to palm away. It's straight at the goalkeeper but is very powerful.
Noble v Jordan Ayew round 2, as the West Ham skipper fouls the Ghanain forward and the pair both get a talking to from the referee.
29'
West Ham enjoying a spell of possession since their change and the defensive shake-up has seen Rice move into the middle. Zabaleta has joined the back three, with Byram now the wing-back on the right.
27'
S. Byram enters the game and replaces W. Reid.
Reid dived into the tackle to block Mawson but it was a really awkward twist from the New Zealand international. A change has to be made and Sam Byram enters the frame. West Ham's defence is likely to be moved about, I'll update on that when it becomes clear what Moyes has done at the back.
25'
Still delayed here and a chunk of added time will be added on at the end of the first half. Reid is just about to be carried off after nine minutes of treatment.
23'
STAT: Back to the goal, it continues a superb run of form from Ki Sung-yueng. After 15 games without a goal or an assist, he's now had a hand in three goals in his last four appearances. Two of those have come from him directly, whilst he's also provided an assist.
21'
And now there's a clash on the field. Andre Ayew is complaining about something and his brother, Jordan, takes matter into his own hands. He squares up with Mark Noble and it's added another talking point to a match that's already catching the eye.
19'
In the aftermath of that, West Ham skipper Winston Reid is down and looks to be in a lot of pain. It really doesn't look good for the central defender and the Hammers may be forced into an early change. A stretcher has just been called by the referee and Moyes will have to shake-up his defensive line.
17'
CHANCE! Swansea agonisingly close to doubling their advantage! The hosts cause problems from the corner as West Ham struggle to get the ball away. It's headed back into the six-yard box by Kouyate and falls kindly for Mawson, though a crucial block is made by Rice. Swansea pounce on the rebound but can't hammer it home!
16'
CHANCE! Ki is holding all the strings! The South Korean captain whips a dangerous cross towards the far post from a free-kick, which is knocked back across goal by Mawson but crucially palmed behind by Adrian for a corner.
14'
West Ham's current scoring run is the second-best in the Premier League, behind only Spurs, but this will be a real test of their character. There's plenty of time left but David Moyes' side cannot afford to lose this game.
12'
Looking at the goal, Adrian probably should have done better. His view was blocked but the shot almost looked like it was slow motion, lacking a lot of power and the Spaniard will surely be disappointed with himself.
10'
That's now 51 goals conceded by West Ham this season, an audacious amount. Swansea will know their work isn't done yet though as the Hammers have scored in each of their last 11 Premier League matches.
8'
Ki Sung-Yueng has scored a goal for Swansea City! Assist by A. Ayew.
Andre Ayew has already provided an impact then against the side he left in January, although it was a simple pass and Ki did all the work.
GOAL! Swansea take the lead! The hosts have started well and Ki has scored from range, cutting onto his left and it trickles into the bottom corner. A fairly tame effort but Adrian had his view blocked, allowing the South Korean to score for the second home game in a row.
6'
There isn't any snow on the field, with Swansea avoiding the weather's wrath, but both sides have been affected by the cold front. Swansea have been training at the Liberty Stadium this week, whilst West Ham had to travel by coach yesterday as their planned flight was cancelled.
4'
The game has started with a very fierce tempo, with both sides not holding back on their challenges. Referee Martin Atkinson is letting the game flow as much as he can.
2'
It looks like West Ham are mirroring Swansea with a back three, using Evra and Zabaleta as wing-backs. Rice and Cresswell play centrally but they're both out of their favoured positions.
1'
KICK OFF! We're underway at the Liberty Stadium, where West Ham get us started and attack to the right in the first half.
Good afternoon and welcome to our LIVE coverage of the Premier League clash between Swansea City and West Ham, a game that will have a huge impact on the relegation battle at the foot of the table.
As mentioned, just three points currently seperate the two sides and a home win for Swansea would give their survival hopes a huge boost. West Ham, with 30 points to their name, know that a victory on the road would put them within touching distance of safety but Swansea's upturn in form will certainly leave them concerned.
West Ham XI: Adrian; Pablo Zabaleta, Winston Reid, Aaron Cresswell, Patrice Evra; Declan Rice, Mark Noble, Cheikhou Kouyate, Manuel Lanzini; Javier Hernandez, Marko Arnautovic.
Swansea subs: Kristoffer Nordfeldt, Luciano Narsingh, Tom Carroll, Leon Britton, Tammy Abraham, Nathan Dyer, Kyle Bartley.
West Ham subs: Joe Hart, Michail Antonio, Joao Mario, Sam Byram, Jordan Hugill, Josh Cullen, Marcus Browne
After a 4-1 defeat to Liverpool last weekend, David Moyes makes three changes to his West Ham side. Rice, Reid and Hernandez all come into the fold, with Collins, Ogbonna and Mario dropping out. It's difficult to assess exactly how they'll line-up this afternoon as it's a very defensive looking contingent.
West Ham have won all three of the last meetings between the two sides but have only won twice away from the London Stadium this season. Sitting just three points above the south Wales side heading into kick-off, there's a lot to play for as a defeat could be catastrophic for the months ahead- but a win would be huge.
Swansea have been rejuvenated under the leadership of Carlos Carvalhal and currently look like the side most likely to pull off this year's great escape. Under the Portuguese boss, the Swans have crucially found some consistency and have won their last three home league matches at the Liberty Stadium. However, their record against West Ham is poor.
Swansea make seven changes to the side that beat Sheffield Wednesday in the FA Cup in midweek but it's a familiar looking Premier League team from Carvalhal, with just two changes made from last week's 4-1 defeat to Brighton. Andre Ayew starts for the first time since returning to Swansea from West Ham in January, whilst Leicester loanee King also features. Dyer and Carroll drop to the bench.
Swansea XI: Lukasz Fabianski; Kyle Naughton, Mike van der Hoorn, Federico Fernandez, Alfie Mawson, Martin Olsson; Andre Ayew, Ki Sung-yueng, Sam Clucas, Andy King; Jordan Ayew.
HALF TIME! After ten minutes of additional time, the whistle goes and Swansea will have their heads on cloud nine. The hosts have been fantastic, especially Ki Sung-yueng who scored the first and provided an assist from the corner for Mike Van der Hoorn. Unfortunately, West Ham lost captain Winston Reid to injury and it looked really bad.
Swansea on course for a club-record seventh win at home across all competitions, which is something they haven't done since moving to the Liberty Stadium 13 years ago. Their defence has largely been untested but that's credit to the work they've done without the ball, as they've put West Ham under a lot of pressure.
A first half to forget for David Moyes' side, who trail Swansea by two goals but, perhaps more concerningly, have lost another defender to injury. Since selling Jose Fonte to China, the Hammers have lost James Collins, Angelo Ogbonna and now Reid. Given that they're being sucked well into the relegation battle as it stands, that could be decisive.
FULL TIME! Swansea make it four Premier League victories in a row at the Liberty Stadium and pick up a vital three points. Ki Sung-yueng was the man of the moment in the first half, scoring the opener and then putting a ball on a plate for Mike Van der Hoorn from a corner. After the break, a set-piece was again the route of success for the Swans shortly after the restart as Andy King scored his first goal for the club, with Jordan Ayew tucking home from the spot- meaning that Michail Antonio's effort later on was a mere consolation.
The victory sees Swansea surge up the Premier League standings, with Carlos Carvalhal's side rising out of the relegation zone and overtaking five teams, including West Ham, in the process. Another crucial relegation fight awaits next weekend against Huddersfield in west Yorkshire but the Swans now seem to have the momentum needed to pull off a great escape.
West Ham, meanwhile, have been sucked back into the relegation dogfight and miss the chance to move six points ahead of the relegation zone. Instead, their advantage over the bottom three is just three points and the fact they now have the joint-worst defensive record in the Premier League is a massive concern. They host Burnley next weekend, who ended an 11-match winless run with victory against Everton earlier today.
