We are into the first of three added minutes. A short corner is worked to Matic and, clearly feeling confident with his recent goals, he lashes a wild effort high and wide.
90'
The delivery is eventually cleared by Gibson. However, Chalobah's long ball puts Boro under further pressure. Costa tries to touch it inside for Fabregas, but he's beaten to it and they have to settle for a corner.
89'
P. Bamford gets yellow.
Bamford scythes down Alonso near the byline and the striker goes into the book as a result.
88'
So, as it stands Chelsea can win the title with a victory at West Brom on Friday! Conte applauds the crowd as they chant his name with the Premier League trophy moving closer.
86'
Fabregas looks to curl a clever pass in behind Gibson for Costa, but once more Guzan comes off his line and beats the striker to it.
85'
Terry's first touch is cheered. Should Chelsea clinch the title, the final day of the season at Stamford Bridge should be quite an occasion for the outgoing captain.
84'
J. Terry enters the game and replaces David Luiz.
A standing ovation for Terry as he comes on for David Luiz to play out the closing stages.
83'
R. Gestede enters the game and replaces Álvaro Negredo.
Agnew responds by swapping Negredo for Gestede.
81'
N. Chalobah enters the game and replaces Pedro.
Conte makes his second change, sending Chalobah on for Pedro.
Minute
Description
80'
Cahill is down and holding his hamstring after an aerial collision with Bamford. He's back to his feet and looks like he'll try to walk it off.
79'
GREAT SAVE BY GUZAN! Willian's low cross to Costa at the near post is blocked, but the rebound falls kindly for Moses. He smashes the ball towards goal with the outside of his right boot but Guzan flings himself in the way of it!
77'
OVER THE TOP! A sloppy Bamford pass is seized upon by Chelsea with Fabregas launching a counter-attack. He plays it out to Costa on the left and he holds the ball up and rolls it on into the path of Pedro, who drills his left-footed effort high of the target.
76'
Willian and Pedro combine neatly, with Costa getting involved at the top of the box. He feigns to shoot before trying, and failing, to bulldoze his way into the box.
75'
Chelsea have slowed the tempo now. They're happy to knock the ball about at ease, keeping it inside the half without rushing forward moves.
73'
Terry sets off down the touchline for a warm-up and is given a rousing reception from the fans.
72'
Willian enters the game and replaces E. Hazard.
That's the end of Hazard's night. He's off for Willian, whose been in impressive form in recent weeks.
70'
There's a break in play with Costa and Guzan down following a collision as the striker chased down a pass in behind. Both are up and don't have treatment, so we will be back underway imminently.
69'
Leadbitter tries his luck from 25 yards with a low shot that Courtois smothers. There are ironic cheers from the fans, with the goalkeeper called into the action for the first time since the restart.
68'
It looks like Boro are destined for relegation tonight, now. They've not offered anything to put Chelsea under pressure at Stamford Bridge, but that's no simple task.
66'
That is Premier League history for Fabregas too. He's the first player to get 10 assists in six separate Premier League seasons! Bravo.
65'
N. Matić has scored a goal for Chelsea! Assist by Cesc Fàbregas.
Fabregas has been phenomenal tonight, and he fully deserves that assist. It was a nice little pass with the outside of his right boot.
GOOOOOOOOAAAAL!! THERE'S THE THIRD! The corner is taken short and Hazard carries the ball in from the right to the top of the box before offloading to Fabregas. He stabs an excellent pass to Matic and he controls on his chest before slotting his effort underneath Guzan.
64'
Moses breezes past Downing on the right and he dribbles into the box along the byline. He powers a delivery towards Costa that hits a red shirt and ricochets out for a corner.
62'
Leadbitter launches a long ball up towards Bamford who cushions it down for Negredo with a header. He has Matic sticking a leg in his way, though, and he's not able to get his effort goalwards.
60'
Fábio gets yellow.
Fabio goes into the book for a cynical challenge on Hazard.
59'
Having navigated their way through that storm of action inside their own half, Boro manage to spend a few minutes in Chelsea's part of the pitch and ease the pressure.
57'
P. Bamford enters the game and replaces Adama Traoré.
Former Chelsea striker Bamford comes on to replace Traore.
56'
G. Leadbitter enters the game and replaces A. Forshaw.
Agnew makes his first change with Forshaw off to be replaced by Leadbitter.
55'
JUST WIDE! Chelsea are pushing harder and harder for a third. Fabregas looses an effort from 25 yards that Chambers sticks his head on and steers it towards the opposite corner. Guzan is wrongfooted but can breathe a sigh of relief as the ball bounces outside the upright.
54'
GREAT HIT! Cahill rifles an absolute piledriver towards the top-left corner from 25 yards! It's on target and there's too much power for Guzan to hold it. Fortunately for him, no blue shirt is on hand to punish him on the rebound.
52'
SO CLOSE! Alonso again races into acres of space on the left and is fed by brilliant footwork from Pedro. He flashes a low cross towards the far post that Costa gets his studs on but fails to steer into the back of the net.
51'
Azpilicueta has been pushing very high up today and he tries his luck with a shot from 25 yards. He hits it well but it flies narrowly over the top-left corner.
49'
Matic tries to feed Pedro in behind the Boro defence after some neat interplay between Azpilicueta, Pedro, Fabregas and Costa. Guzan comes off his line and gathers comfortably.
48'
Boro have a chance to swing the ball into the box from a free-kick, but the referee's assistant puts his flag up once Downing's delivery floats in.
46'
We are back underway!
OFF THE BAR AGAIN! Just as they did in the first half, Chelsea rattle the woodwork soon after the restart. Hazard rolls the ball across the top of the box to Pedro, whose powerful shot smashes the top of the bar and flies into the stand behind the goal.
45' +1
There is the half-time whistle!
45'
We are going to have one minute of added time at Stamford Bridge.
43'
Matic launches a quick break through the middle, spreading the play out to Hazard on the right. He dribbles inside but sees his options limited, and his attempt to tee up Matic for a 20-yard shot is intercepted.
41'
ALONSOOOO!! The left wing-back attempts to curl a shot into the top-right corner and a deflection sends it out for a corner. Fabregas takes it and David Luiz angles his header well wide.
40'
Costa is fouled by Forshaw and Chelsea have a free-kick in another dangerous position, this time a bit closer and David Luiz has walked away from it.
39'
STAT: Fabregas is one assist away from becoming the first player to register 10 in six separate Premier League seasons.
37'
MOSES! The wing-back is released by Fabregas, once more given far too much time and space to pick his pass by Boro. Moses rushes into the box but produces a weak shot that Guzan saves comfortably, sending it behind with his legs.
36'
Given the angle of Alonso's shot and the deflection off Guzan, that could well go down as an own goal eventually.
34'
Marcos Alonso has scored a goal for Chelsea! Assist by César Azpilicueta.
GOOOOOOAAAAAL!! ALONSO MAKES IT TWO! It's the same ball that undoes Boro again! This time it's Azpilicueta who finds Alonso making a run inside of Fabio. He drills the ball into the centre from an acute angle and it bounces off the inside of Guzan's leg and finds the back of the net!
It was a brilliant ball from Azpilicueta, cutting Boro apart in their weakest area.
33'
Traore drifts into the centre and tries to go it alone and dribble through the heart of Chelsea's defence. He's swarmed upon by Blue shirts and Matic puts a challenge in to deny him.
31'
OPTA STAT: No player has scored the opening goal of a Premier League game this season more times than Diego Costa (7).
29'
Chelsea are awarded a free-kick on the right but this time Fabregas' delivery is poor. It's flat and headed clear by Negredo.
27'
David Luiz stoops awkwardly to try and intercept Downing's cross from the left and he inadvertently lets it run through to De Roon. He shifts the ball onto his left foot and cracks a shot that is blocked by Cahill.
25'
Traore shows the sort of spirit Boro are going to need tonight by closing down Cahill and Alonso deep in Chelsea's half. He manages to get a touch on the ball to send it out of a throw-in, though it doesn't take long for the Blues to work their way free.
23'
Diego Costa has scored a goal for Chelsea! Assist by Cesc Fàbregas.
GOOOOOOOAAAAAAL!! COSTA OPENS THE SCORING!! Once more it's a diagonal ball into the left-hand side of the box from Fabregas is not dealt with by Fabio. Costa takes full advantage, taking a touch before slotting his 20th Premier League goal of the season between Guzan's legs.
22'
Negredo holds up the ball and sets Downing on his way with an excellent pass out to the right. Moses gets back and does enough to slow the momentum of the attack, with Clayton dispossessed as Boro look for a route through the centre of the defence.
20'
WHAT A CHANCE! Azpilicueta's excellent long pass is controlled exquisitely by Hazard on the left-hand side of the box. He rolls a pass through the legs of Fabio and across the face of goal for the run of Costa. It's out of the striker's reach, and his claims he was fouled by the tracking Friend are ignored.
19'
Chelsea have Boro pinned back inside their final third. Moses' ball from the right is repelled and so the Blues move it out to Alonso on the left. He tries a one-two with Cahill, whose return pass goes out of play.
17'
FABREGAS! Cahill wins possession near the halfway line and Chelsea break with Costa releasing Hazard down the left. He gets to the byline and gets his head up, biding his time before rolling a pass back to Fabregas at the edge of the area. The Spaniard tries to guide a first-time finish into the bottom-left corner but gets his aim wrong and sends it wide.
15'
David Luiz takes the free-kick but it flies harmlessly over the top. He clearly hasn't found his range yet.
14'
Chelsea step up the pace with a series of quick passes, Hazard dummying the ball for Costa who is chopped down by Fabio in a central area 30 yards out. David Luiz might fancy this.
12'
Azpilicueta flights a decent ball forwards for the run of Costa. The Spain international jumps and stretches a leg to try and bring it down in the area but he's unable to get it under control.
10'
Chelsea again look to Alonso down the left but with Middlesbrough defending in numbers he looks inside to Matic. The midfielder attempts a first-time pass towards the top of the box that goes straight to a red shirt, drawing groans from the stands.
8'
Traore has been poor defensively so far but he shows his attacking prowess now, driving to the byline and firing a low cross into the middle. Forshaw's touch is heavy but he manages to force a corner, which Downing takes and sees cleared away.
7'
Fabregas clips another sublime diagonal pass out to Alonso on the left-hand side of the box. Rather than shoot he tries to square for Hazard, but his cross is overhit and only finds Moses near the corner.
5'
Middlesbrough finally get out of their half but Traore fails to create anything on the right when he comes up against Azpilicueta. The early signs suggest the visitors are going to be in for a long night.
3'
Chambers pulls Costa back on the left and gives Chelsea a free-kick. Fabregas swings it towards the back post and it's too high for the head of David Luiz, bouncing behind for a goal-kick.
2'
OFF THE BAR!! What a start from Chelsea! Alonso is left unmarked at the back post and he fires a volley across goal from Fabregas' clipped delivery. He hits it into the ground but Guzan appears to get a touch on it to divert it up onto the woodwork!
1'
Boro get the ball rolling at Stamford Bridge!
It's a big night in the respective Premier League seasons of Chelsea and Middlesbrough at Stamford Bridge, and you can follow all the action right here.
After Tottenham's defeat at West Ham on Friday, Chelsea have a chance to open up a seven-point lead at the Premier League summit with just three games left to play, bringing the title even closer to a return to west London. The omens favour them tonight; Chelsea have won each of their last six top-flight matches against Boro without conceding a single goal.
Middlesbrough, meanwhile, are playing for Premier League survival. Lose tonight and they will be relegated. A draw will keep their hopes alive for another week, but it looks pretty bleak for Steve Agnew's side down in 19th.
Chelsea (3-4-3): Thibaut Courtois; Cesar Azpilicueta, David Luiz, Gary Cahill; Victor Moses, Cesc Fabregas, Nemanja Matic, Marcos Alonso; Pedro, Diego Costa, Eden Hazard.
The teams have been confirmed so let's see how they will line up.
Chelsea bench: Asmir Begovic, Kurt Zouma, Nathan Ake, John Terry, Nathaniel Chalobah, Ruben Loftus-Cheek, Willian.
Middlesbrough (4-3-3): Brad Guzan; Fabio, Calum Chambers, Ben Gibson, George Friend; Marten de Roon, Adam Clayton, Adam Forshaw; Adama Traore, Alvaro Negredo, Stewart Downing.
Middlesbrough bench: Dimitrios Konstantopoulos; Antonio Barragan, Bernardo, Grant Leadbitter, Adlene Guedioura, Patrick Bamford, Rudy Gestede.
Antonio Conte makes just one change with a muscular issue for N'Golo Kante seeing Fabregas promoted to the starting line-up. There is also just a single switch for Agnew, with Traore returning to the starting line-up in place of Cristhian Stuani.
Kante was named Player of the Year by the Football Writers' Association today and will be a big miss for Chelsea. Matic will be relied up on to try and have the disruptive impact the Frenchman so often does on opponents, though it will be tough to try and match the energy and dynamism the former Leicester City man provides.
The teams are heading out onto the pitch, so kick-off will be upon us in just a few moments.
HALF-TIME - Chelsea 2-0 Middlesbrough: Goals from Costa and Alonso have Conte's men close to three points that would put them one match away from clinching the title. They've dominated the first half with Fabregas pulling all the strings. Boro need to step it up after the break if they are to find some way of avoiding relegation tonight.
That's all we have time for tonight. Thanks a lot for joining us!
FULL-TIME - Chelsea 3-0 Middlesbrough: Just three points separate Chelsea from the Premier League title after goals from Costa, Alonso and Matic. It was a routine performance for the Blues, who dominated from start to finish. It brings an end to Boro's stay in the top flight, with another season in the Championship awaiting them.
